Why is this not simpler, like a form I can submit to my child’s teacher?

We have conducted a survey of our participating schools and found that each district has different privacy regulations or preferences regarding whether school staff are allowed to inquire as to vaccination status or collect vaccine information. In order to allow the maximum number of schools to participate safely and respect districts’ policies regarding vaccine data collection, we have removed the process from the purview of the advisors with the exception of letting them know if a student has yet to fill out this form.
